AI medical chronology
A date-ordered view of the medical record, with a path back to the evidence. CoClerk helps IME, healthcare and legal teams turn scattered documents into a structured timeline for professional review.
What is an AI medical chronology?
A medical chronology arranges relevant events from a person’s medical records in date order. Instead of reading a file in the order documents arrived, a reviewer can follow consultations, investigations, treatment and changes in function over time.
AI assists with extracting and organizing that information. A useful chronology preserves the event date, provider, documented findings and a reference to the original record. It is a working summary of the evidence—not a new medical opinion, diagnosis or legal conclusion.
For an IME, personal injury or long-term disability matter, this can help a team get oriented before reviewing the underlying records and preparing questions or a report.
What a chronology should contain
Dates, provider names, relevant encounters, tests, treatment and source references belong together. Missing dates, conflicting accounts and gaps in the supplied records should be flagged for review rather than filled in by assumption.

Medical chronology within your wider workflow
CoClerk is Kolabrya’s automation system for IME, medical and healthcare firms. Medical-record organization is one part of a wider lifecycle, from referral and record collection to report drafting.
Source-linked summaries
Organize clinic notes, hospital charts, imaging reports and specialist correspondence into a structured chronology with references back to source pages.
Clinical timelines
Bring together dates of loss, first presentations, diagnostic imaging, treatment blocks and return-to-work milestones for review.
Drafting and review
Use the organized record to support draft IME report sections. Your team checks the evidence and retains responsibility for the final report.

Professional review stays essential
AI can omit a detail, misread a scan, confuse an encounter date with a document date or produce a reference that does not support the summary. Before relying on an entry, compare it with its source record.
- Confirm the patient, date, provider and page reference.
- Separate what a patient reported from what a clinician observed or concluded.
- Check duplicate records, missing pages and conflicting dates.
- Do not treat a gap in supplied records as proof that no care occurred.
- Leave causation, prognosis and legal interpretation to the appropriate professional.
